Caringly reduces CMS penalties, improves patient safety, and enhances clinicians lives by making it easy to exchange the right patient information between the right providers at the right time.

Diagram. Circle in the middle has a heading of Contextual Patient Information followed by 3 boxes below: Clinical information for residents, nurses, and hospitalists; Educational information for social workers, and Financial information for case managers. Outside of the center circle is a donut shape with handoffs, transfers, rounding, and discharges. There are five squares outside of the donut each representing the Resident, Nurse, Hospitalist, Case Manager, and Social Worker.

Products

Different teams have different needs for transitions of care. We’ve designed unique products that meet the needs of nurses, hospitalists, residents, case managers, and social workers. We also offer Caringly MDR, which facilitates accurate and convenient information exchange during interdisciplinary rounding.

How It Works

Miscommunication during patient care transitions heightens the risks of serious medical errors and increases clinician workload, which leads to burnout.

Caringly is the only platform that lets you measure and improve the quality of information exchange during care transitions such as handoffs, transfers, rounding, and discharges. We improve information exchange through intelligent prioritization of clinical data, appropriate guidance, standardized workflows, and an integrated display of patient data.

Diagram with 3 main parts, the Provider on the left, which is connected to the AI Rules Engine in the middle, which is connected still to the Context on the right.

The Provider can be a nurse, resident, hospitalist, social worker, or case manager.

The AI Rules Engine has a list underneath that says Intelligent Prioritization, Appropriate Guidance, Standardized Workflows, and Integrated Patient Data.

The Context has a list underneath that says Transfer, Handoff, Rounding, Discharge, and Information Only.

Intelligent Prioritization of Clinical Data and Tasks

Caringly’s patent pending algorithms make it easy to prioritize tasks and information that needs to be communicated in the form of checklists, flags, and reminders.

This guidance is tailored to the nurse, hospitalist, resident, case manager, or social worker who’s using the application.

Two screens. One screen shows a label "Curated by AI Rules Engine" and sections corresponding to Illness Severity, Actions, Vitals, Metabolic Panel, and Problems. The Vitals section shows vitals such as Temperature, Heart Rate, Blood Pressure, and so on.

Another screen shows a title Case Manager Handoff. In the content section there are cards labeled 1 North ICU and 1 South ICU with count indicators of 10 and 8 respectively. In the 1 North ICU card there is an alert for 4 high risk and in the 1 South ICU there is an alert of 3 Transfers RN.

Appropriate Guidance

Create effective clinical learning environments for new-to-practice nurses and residents. Caringly provides dynamic checklists, step by step guidance, and structured processes for providing feedback on care transitions.

3 computer screens laid on top each other. One screen shows some reminders for a Nurse handoff: "Before you accept this handoff please make sure you ask the following questions, is the patient a fall..., is the bed alarm.... Another screen shows survey questions with a heading of Handoff Review. Outgoing resident Ricardo dela Cruz, incoming resident Mei Chen, Handoff giver quality, situational awareness, mnemonic frequency and so on. Finally there is a confirmation dialog that asks "Of the 9 patients in your list, you have addeed Notes for 3 patients. Please add Notes for these patients before completing handoff."

Standardized Workflows Without the Extra Work

Reduce the need for overtime work, delays, and rework with Caringly’s efficient information entry and standardized care transition workflows.

Caringly seamlessly integrates patient handoffs, transfers, rounding, and discharges into the clinician’s workflow without negatively impacting their workload.

Two screens. One screen shows the patient information of Frida Moreno including an edit box for Handoff Summary, information displays of Illness Severity, Vitals, and Metabolic Panel.

Another screen shows handoff of patient Vince Vaughn with visible sections for Situation, Background. Under Situation, there is some patient data and notes on CHF Exacerbation, Allergies, Core Measure and a checkbox that says the information has been shared.

Integrated Display of Patient Data

Information overload causes clinicians to overlook important information, especially when working on EMRs.

Caringly’s user interface integrates relevant patient data that would typically be scattered across different tabs in an EMR into a single display that provides a broad overview of the patient’s condition, which includes their current hospital stay and previous medical history, as well as a focused view of critical information about each patient’s condition.

Screens that show patient data such as illness severity, actions, vitals, metabolic panel, problems and allergies, medications in a single place. There's also a label that says Curated by AI Rules Engine.

Dashboard with different sections. There's a section for Care Transition Quality Index and a score of 85 meaning great. Another section shows the Most Common Diagnoses with Summaries, with a list that includes Septicemia 116, Osteoarthritis 112, Complications during childbirth 112, Acute myocardial infarction 49, Cardiac dysrhythmias 22.

Another section shows Employee Time Spent with a list of employees and the hours and minutes they've spent.

Care Transitions Made Measurable

Can you answer the following questions?

  • What was the quality of patient handoffs at your hospital this morning during nurse shift change?
  • How much time did your residents or nurses or case managers spend in handing off patients today or last week?
  • How many handoffs contain incorrect information?
  • Which staff members perform the best handoffs?

If these are challenging to answer, then transitions of care within your facility are not quantified. And without the data to know if they’re effective, you don’t have a systematic means to make improvements.

Caringly Q helps you answer these questions and more by measuring the quality of information exchange through shift summary reports, length of stay metrics, care transition performance surveys, and real-time KPIs.

Integrations

Caringly supports two-way integration through HL7 or FHIR feed with all EMRs. Caringly helps avoid double documentation while retaining the EMR as a single source of truth.
